Upward Transitions serves those who are homeless and on the verge of becoming homeless by providing case management and stabilizing resources to meet basic social needs. The ultimate goal is to assist individuals and families to reach their potential with the ability to support themselves and contribute back to their community.

Upward Transitions is committed to community development and advocacy for those who are poor and homeless.

New Clients seeking assistance with Legal Documents & IDs may Walk-in with a Verification of Homelessness and Referral.

Monday – Friday 1 p.m.-3:30 p.m., Appointments Only:
Current Clients may make appointments with their case managers in advance.

New Years Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day,
Thanksgiving Day, Day After Thanksgiving, Christmas Eve, and Christmas Day.
