Community Outreach

Our services provide assistance to families and individuals who face threats to financial stability. In many cases, Upward Transitions can provide a short-term financial bridge in the form of emergency assistance and access to community resources to maintain stability. Eligible families may work with case managers on a long-term basis to increase and develop skills that can provide a sustainable income. Case managers work closely with these families to create a plan with goals that address specific needs and create the means to avoid future homelessness.

Homeless Prevention

Utility Assistance

Must live in Oklahoma County.
Have a cutoff notice in the client’s name for the current address.
Have minor children under the age of 18 or senior 55 years of age or older living in the home.
Have proof of hardship which has caused difficulty in paying the bill.
Have proof of income or pending income.

Rent Assistance

Must live in Oklahoma County.
Have a 5-day notice to quit or eviction notice in the client’s name for the current address.
Have minor children under the age of 18 or senior 55 years of age or older living in the home.
Have proof of hardship which has caused difficulty in paying the bill.
Have proof of income or pending income.

Homeless Services

We are working with our partner agencies to provide for the needs of our mutual clients without the client having to come to Upward Transitions in person. Individuals who are homeless should discuss the need for a referral to Upward Transitions services with their case manager, advocate, or counselor at the shelter or transitional housing where they reside or agency which provides them with healthcare, mental healthcare, rehabilitative services, etc.

Individuals who are homeless and working to regain their independence can turn to Upward Transitions for help.

Our services provide the following forms of assistance:

Legal document retrieval assistance for clients who are homeless (birth certificates, marriage licenses, etc.)
State ID purchase assistance for clients who are homeless
Public housing rent deposits (Call to confirm that funds are available.)
Linkage to other services

Verification

An Upward Transitions’ Partner Agency must email our Verification of Homelessness and Referral to us for any individual seeking the following services: birth certificate retrieval, obtaining state ID, and transportation for employment purposes.

Ways to attain a referral:

If you reside in a shelter, please contact your case manager at the shelter for a referral.
If you reside on the streets or in an abandoned building, tent, or car, please contact an agency, day shelter, or service provider which can verify your homeless status and provide a referral to us.
If you reside in a transitional housing program, please contact your advocate in the program for a referral.
If you reside with others temporarily (1-2 months not paying rent), please contact an agency, day shelter, or service provider which can verify your homeless status and provide a referral to us.
If you have recently been released from incarceration please contact your case manager, parole officer, or advocate for a referral.
